Application Forms - In PDF format. Note there are two options: print blank form and fill the form in online before printing.

Option 1: Print as many copies of the blank forms below as required.

Option 2:

(a) Complete the form online then print before passing it to the DDGM
(b)  Complete the form online, save to your computer, then email to the DDGM as an attachment.
(c)  The DDGM fills in his portion online and emails to the Chairman of the Awards Committee.

It should be noted that any member of the Lodge can nominate a brother for the Quinn Medallion. Fill in the form, ensure it is signed and forward to the DDGM within the deadline. The Committee will not consider late applications

To All District Deputy Grand Masters of the Grand Lodge of New Brunswick
Your duties regarding Grand Lodge Awards - Quinn Medallion and Beardsley Medal

It will be your responsibility to:
  • Promote these Grand Lodge Awards at every convenient opportunity. You should remind the Lodges that if an application of a brother has been previously submitted and that brother did not receive an award, the Lodge, or member, may choose to re-submit an application on behalf of this brother.

  • Ensure the nomination of a brother is eligible for the Award applied for and each application is properly completed.

  • Ensure that each applicant for the Beardsley Medal has been submitted by a voting member of this Grand Lodge.

  • Complete the "Comments of the District Deputy Grand Master" area of each application.

  • Forward all applications to the Awards Committee, care of the Grand Secretary, postmarked not later than March 7, 2010.
  • Must be in hands of DDGM no later than March 1, 2010

  • Note:- consideration will not be given to applications received after this closing date.

  • All applications expire annually and must be resubmitted if not successful.

    Beardsley Medal

    The Beardsley Medal is given to any member of this Grand Lodge who has made an exemplary contribution to the Grand Lodge of New Brunswick and the advancement of Freemasonry.

    This Medal is awared to a member of the Grand Lodge in recognition of praise-worthy service rendered to the Grand Lodge of New Brunswick

    The application must be submitted by a voting member of Grand Lodge on behalf of a worthy Brother. Each completed application is forwarded to the DDGM no later than March 1, 2008. After adding your comments, please ensure the the application is addressed to the Awards Committee and sent to the Grand Secretary no later than March 8, 2008.

    No more than two will be given each year in the entire Grand Lodge jurisdiction if worthy candidates are presented.
    Quinn Medallion

    The Quinn Medallion is given to any Master Mason or Past Master who has served his lodge with honor and dedication over a period of years.

    This medallion represents the award the Blue Lodge can use to honor a member who has given long and dedicated service to his lodge. The recipient will be a member who has, through his work and dedication, improved the quality and performance of the Lodge.

    Any member of the Lodge may submit an application on behalf of a worthy Brother. Each completed application is to be forward to you. After adding your comments, please ensure the the application is addressed to the Awards Committee and sent to the Grand Secretary not later than March 8, 2008.

    The present number of one per district will be given if worthy candidates are presented.
